Atrial septal defect on chest radiograph

CASE

The chest radiograph shows dilated truncus intermedius, somewhat increased vascular markings, heart silhouette dilated to the left with apex pointing laterally suggesting dilatation of the right ventricle. Enlarged bulge of pulmonary artery can be appreciated between the left hilum and the heart.
