Extensive sequela of pulmonary embolism - large chronic clot in the right pulmonary artery and preobliteration of the common branch for basal segments of the left lower lobe. Note also mosaic perfusion, enlargement of the right heart ventricle with paradoxical movement of interventricular septum, reflux of contrast into hepatic veins and the azygous vein.
