Ultrasound in this patient repeatedly showed hyperechoic liver parenchyma with attenuation of ultrasound beam towards the periphery consistent with liver steatosis. A focal well-defined lesion in the parenchyma close to portal vein has somewhat atypical shape, it is hypoechoic, however with a slight increase of echogenicity in its center. A differential diagnosis would include solitary liver metastasis and focal sparing of the liver which proved ultimately correct and the lesion remained stable over several follow-up examinations.
